How to Request Your Medical Records

In accordance with HIPAA regulations, you may request copies of your personal health record.

Request your health information for yourself, a family member or caretaker, a healthcare provider, or anyone else you wish in the Sharing Hub on MyChart.

Complete a Printed Form

Complete and submit the written Request for Medical Records Form.
