What do I need to bring to my appointment?

Please bring an updated medication list, a photo ID, insurance card and a list of questions you may have for the provider. If you are new to the clinic a copy of your previous medical records. If you have any previous x-rays or scans from another location, please bring them to your appointment.

Will I have surgery the day of my first appointment?

Your first visit is most likely for a consultation only. However, if it is an emergent condition it is possible that you will have surgery that day.

Do I need a referral?

You need a referral for any spine appointments or if your insurance requires it. If you have further questions please call the Orthopedics Department at 515-239-4475.

Do I need to contact my insurance company before I have surgery?

Please contact your surgeon's nurse, as they can best assist you with this question.

Do I need to come early to my appointment?

Many patients upon arrival need to complete forms or need additional orthopedic x-ray views.
