Respirator

For the provider to qualify the employee to wear a respirator, we need to know the type of respirator(s) the employee will be required to wear. Initial qualifications include:

1. “Hands on” physical examination
2. OSHA questionnaire completion
3. Pulmonary Function Test (PFT)/Spirometry
